No cost if your practice has Rural Health Transformation money

Your notes shouldn't come home with you.

We help small practices and rural hospitals in New Hampshire fix their tech, get paid for the work they already do, and stay safe from hackers. If you got Rural Health Transformation funding, it's free.

No IT department? That's most places we work. We'll figure it out with whoever's been handling it, even if that's you.

What we help with

The three things we get called about most.

Getting paid for coordination

Your nurses already call folks after they get out of the hospital, track down referrals, and help people find a ride to Lebanon or Concord. A lot of that is billable. Most small practices just never get paid for it.

  • Matching the coordination work you already do to the codes that pay for it
  • Getting the Medicaid enhanced payments you qualify for
  • Social needs screening and referrals your staff can actually keep up with

Your EHR and AI tools

We don't sell software, so we'll tell you straight what's worth it and what isn't.

  • Cleaning up EHR workflows so charting takes fewer clicks
  • Getting an AI scribe set up and actually used
  • Risk scores and decision support that fit the way you work
  • Getting your systems to share data with hospitals, labs and the state
  • Choosing and setting up a population health tool

Keeping your practice secure

Ransomware crews go after small healthcare outfits because they're easy. We start with the fixes that do the most good for the least money and staff time.

  • Your required HIPAA security risk assessment, plus a plan to close the gaps
  • An incident response plan your staff has actually practiced
  • Help picking an outside security vendor, if you need one
  • Multi-factor login, access controls and device protection
  • Backups, and a plan for keeping care going if your systems go down

How we work

Built for places where the office manager is also the IT department.

That's most practices we know. So we keep it simple, and we plan for the day we're not around anymore.

  • One point person

We work closely with one person on your team, so the know-how stays in your building.

  • We teach as we go

By the end, your point person can train the next new hire without us.

  • It's all yours

Every plan, checklist and document we make, you keep and can change.

  • Nothing to sell you

No vendor pays us for recommending their product.

  • We'll come to you

Most of it happens over video. When it needs to be in person, we'll drive. Colebrook included.

  • We call back

You'll hear from us within three business days.

An example

Say the computers go down Monday morning. Then what?

Most practices have a plan in a binder somewhere. Hardly anybody has practiced it. We write you a short one with a name next to every job, and a backup for when that person's out. Then we sit down with your team for an hour and a half and walk through it like it's really happening.

At every step we ask the same four things. Who makes the call? Who does what? Who tells staff and patients? And what are your patients going through right now?

What you end up with

  • A short plan, 10–15 pages, with a name next to every job
  • Laminated "first hour" checklists to keep by the phone
  • A simple guide to who you have to notify, and by when
  • A downtime kit so you can keep seeing patients on paper
  • A short write-up within a week, and a 90-day to-do list with owners

Who we are

Two of us. That's the whole firm.

We've worked together since 2019, and we've built and run the same kinds of systems we'll be helping you with. When you call, you get one of us, not somebody we hired last week.

Getting started

It starts with one email.

The Foundation for Healthy Communities runs a program that pays for outside help for organizations getting Rural Health Transformation money. We're one of their approved consultants, so you can ask for us by name.

  1. Email the Foundation

Send a note to GO-NORTH-FHC@healthynh.org. Tell them what's going on and that you'd like to work with Granite North Advisors.

  2. We'll give you a call

We'll figure out what's bugging you most, decide what to fix first, and take care of the paperwork.

  3. We get to work

Once the Foundation signs off, we start. The program pays us. You won't get a bill.

We work with rural health clinics, FQHCs, community health centers, independent practices and critical access hospitals. Not sure you qualify? Email us and we'll find out for you.
